About Appointments

  • You will require a referral from another physician (Family doctor / Specialist) or a Nurse Practitioner

  • Appointments can be cancelled up to 2 business days before the appointment

    Cancellations with in 2 business days of the appointment and no-shows will be subject to a fees.

    No-show after 2 appointments will require a new referral and will be added to the waitlist.

Preparing for the Appointment

  • Arrive 15 minutes before your appointment

    Bring your valid Health Card

    Bring the medications or an up to date list from your pharmacy / your blister pack

    You will get a separate e-mail asking you to complete some forms prior to your appointment

    Inform of your allergies

    Please avoid any scents / perfumes for your visit

  • Video appointments will be done through the patient portal.

    Please allow for a +/- 15 minute window for video visits / telephone appointments

  • You are welcome to have your family / caregivers accompany you for the appointment if you prefer to.

Patient Portal

  • Patient Portal provides you free online access to securely connect with the clinic, send and receive information / documents / reports / forms as well as pay any outstanding invoices.

    Patient portal is provided through Telus Collaborative Health Record (CHR). Telus CHR is the secure electronic medical record used by NEO Clinic.

    • View new and existing messages from your clinic

    • Respond to messages, if enabled

    • Receive PDF copies of labs, forms, handouts and other chart information sent from your clinic

    • View past and future appointments

    • Self-register as patients

    • View and update demographic information

    • View and pay your outstanding bills

Fees

  • Consultations and services are insured by OHIP if you were referred by a health care professional

  • OHIP doesn't cover all services provided in the clinic like

    • sick notes

    • letters / forms

    • reports to insurance / disability

    These services are charged directly to the patient based on recommendations by Ontario Medical Association (OMA)

  • We accept payments through online services including: Credit, Debit, Apple Pay, Google Pay.

    All uninsured services must be paid before the service is provided

    We use the Square platform for invoices and online processing.

  • No show fees will be charged as below

    $200 for New Appointments

    $100 for Follow up appointments
